DEPARTMENT SUMMARY: St. Charles Medical Group encompasses practices in Central Oregon counties and numerous specialties including, family care, internal medicine, obstetrics and gynecology, pediatrics, immediate/urgent care, cardiology, pulmonology, rheumatology, general surgery, cancer care, and sleep medicine. We encourage collaboration between clinical and non-clinical staff to assure we are providing our community with comprehensive and compassionate health care.
POSITION OVERVIEW: The Registered Nurse (RN) – Heart and Lung Specialists provides quality patient and family centered care as a member of the health team in accordance with St. Charles Health System mission, philosophy, policies, and procedures. The RN applies standards of professional nursing practice and focuses clinical care to enhance the experience for the patients, families, and all others. The RN is responsible for daily patient flow. They use triage skills to make appropriate patient health assessments and anticipate physician’s needs regarding patient care. This position functions as primary liaison between patients, staff, and physician. This position supports Cardiology and Pulmonology in a triage environment.
This position does not directly manage any other caregivers.
Essential Functions And DutiesConducts in clinic and phone triage of patients including urgent/emergent walk-ins; makes recommendations for care utilizing approved standing orders and protocols. Communicates (both written and orally) effectively maintaining professional relationships with caregivers, medical staff, patients, and guests.
Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques and applies this knowledge in accordance with St. Charles Health System policies and work instructions and within laws governing nursing in the State of Oregon.
Uses critical thinking to follow physician approved protocols for prescription refills, medication titration and home treatment; utilizes appropriate physician consultation to work within the scope of practice.
Assesses the patient’s condition and needs using critical thinking and clinical judgement; implements appropriate nursing actions and collaborates with other disciplines to meet the patient’s/family’s physical, emotional, and other needs; and evaluates patient’s progress.
Performs EKGs, Holter and cardiac monitor placement, education and follow-up, basic EKG, and rhythm interpretations
Assists patients with scheduling acute or routine appointments, educates patients regarding abnormal test results and/or medical conditions, and explains medical terms and procedures.
Demonstrates independent critical thinking with the ability to multi-task in various departments including urgent situations, conscious sedation, giving work direction, and prioritizing work.
Manages medication administration, actions, reactions, and discharge teaching using the “5 Rights” of medication administration.
Collaborates with healthcare providers and community partners to develop an evidence-based, comprehensive individualized plan of care. Interventions are patient centered and facilitate effective and efficient medical utilization.
Develops, documents, and monitors patient/family response to plan of care, and revises the plan as indicated. Provides patient self-management support and tools with a focus on empowering the patient/family to build capacity for self-care.
Participates in multidisciplinary team conferences, and in overall program evaluation of processes. Identifies problems and proposes/develops process improvement strategies to enhance care management services.
Acts as a clinic resource to acute care departments and referral agencies to help develop best practice and up to date education.

Provides a therapeutic presence in service to others by purposefully responding to the needs of patients in a caring way, including introducing oneself and explaining role in patient’s care, asking the patient his or her preferred name, sitting with the patient to determine his or her care goals, active listening, and communicating effectively and appropriately through touch, eye contact, etc.
Provides and maintains a safe environment for caregivers, patients, and guests
Documents all patient care with proficiency in compliance with hospital policies, procedures, and regulatory agencies.
Calculates dosages and administers medications accurately in accordance with St. Charles Health System policies and procedures for medication administration.
Complies with all applicable laws, regulations, policies, procedures, supporting the health system’s corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all company investigations and proceedings.
May perform additional duties of similar complexity within the organization, as required or assigned.
Licensure/Certification/RegistrationRequired: Current Oregon RN license. AHA Basic Life Support for Healthcare Provider certification. Valid Oregon driver’s license and ability to meet SCHS driving requirements. Ability to travel to business functions/meeting with clients and providers/attend trainings/meetings and all SCHS worksites.

ExperienceRequired: Minimum one (1) year nursing experience in a cardiology or pulmonology setting.
Preferred: Experience in case management, nurse navigation, and/or care coordination. Community Health experience.
Personal Protective EquipmentMust be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
Physical RequirementsContinually (75% or more): Standing and walking, keyboard operation, use of clear and audible speaking voice and the ability to hear normal speech level.
Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds. The use and operation of a motor vehicle for Home Health and Wound Caregivers.
Occasionally (25%): Bending, stooping/kneeling/crouching, climbing ladder/step-stool (varies by area), reaching overhead, lifting/carrying/pushing or pulling 25-50 pounds, grasping/squeezing, ability to hear whispered speech level.
Rarely (10%): Climbing stairs.
Never (0%): Climbing ladder/step-stool (varies by area), operation of a motor vehicle.
